How do I request my own medical records as a client?

Your law firm invites you to MedicalEase first. Once you create your account, go to your Dashboard and select Request Record on the right-hand side. Choose Self Upload to add the documents yourself, or Request from Facility to have MedicalEase contact the provider directly, then select the date range and record type, and select Submit.
